New Patient Information at Visit
On your 1st visit, you will need to bring a drivers license and insurance cards. We also need to know the patient's Social Security Number. Please bring a list of medications the patient is currently taking. If the patient is a minor, a parent must be present, no exceptions. Please call to cancel appointments within 24 hours of your appointment. We have a no call/no show policy, we will not re-schedule. We treat allergy and asthma patients, please do not wear perfume, cologne, body sprays, or scented lotions.
Patient Forms
The Family Allergy & Asthma Center provides our patients with office forms for your convenience. We recommend you print and complete these forms prior to your appointment to save time.
Cancellation Policy
With the exception of serious emergencies, it is expected that you keep all of your appointments. If you need to reschedule an appointment we require 24 hours notice. In such a case, please call our office and arrange for a make-up appointment with one of our patient care coordinators.
